The post-transplant clinic is an integrated group of services that includes clinical monitoring performed on patients at their normal visit, also physiological monitoring performed by laboratory tests including monitoring the immunological aspects of organ tolerance to help detect any impending early rejection and to expedite diagnosis and treatment of primary disease by providing medication reconciliation and treatment just after the patient had their physician's visit and laboratory testing done .

Behavioral and mental support will also be available to family and patient in order to mitigate any issues on a scheduled basis. Thereby maintaining a constant level of concerted and informed patient follow up.

By providing clinical, laboratory, behavioral and pharmacy services, the patient will have covered all needs for monitoring and treatment during the same visit. These visits will occur once a month or as needed according to patient’s clinical, mental, and laboratory testing evolution.
